Clause 4 is the QMS foundation. Done badly, the whole system stays generic. Here's how to ground it in an industrial SMB.
ISO 9001:2015 introduced clause 4 "Context of the organization" as the mandatory starting point of the QMS. Many plants resolve it with a two-page SWOT that nobody revisits. That document won't survive a serious audit. Real context is a live input to decisions.
Clause 4 has four sub-requirements:
All four are assessable. The auditor doesn't look for a document titled "Context." They look for evidence that these factors drive QMS decisions.

A useful analysis doesn't list 50 factors. Focus on the 8 to 10 with the greatest impact on product quality and customer satisfaction.
The common mistake is listing "customers, suppliers, employees, government" without specifying what they expect. A useful matrix has three columns:
Example: the Tier 1 automotive customer expects PPM ≤ 50, which maps to a production-process indicator with an alert threshold at 30 PPM. That's tracing an expectation down to an operational control.
Clause 4.3 requires defining which products, services, sites and activities the certification covers. It's not a formality. A poorly defined scope can invalidate the certification in front of a customer who audits.
Recommended wording: "Design and manufacture of [product] at [location], including [critical processes]." Avoid "all company activities" unless they really are all.
The standard doesn't specify a frequency, but good practice is quarterly. Environmental changes (new big customer, new regulation, new plant) should be reflected in the analysis before the next internal-audit cycle.
Gather leadership, quality, operations and sales. In 90 minutes, update internal and external factors with real data from the last quarter. If the resulting document doesn't look like the previous one, that's a good sign. If it looks the same, you probably weren't looking.
Clause 4.2 requires identifying interested parties and their needs. With AI you can do it in an afternoon, not a month.
